Guidelines for pharmaceutical care in Poland. Pharmaceutical care defined by Hepler as the responsible provision of drug therapy for the purpose of achieving defined outcomes that improve a patient's quality of life should be implemented into the pharmacy practice in Poland. To assess and control the quality of pharmaceutical care delivered in Poland the pharmaceutical care guidelines are needed. The article describe the proposition of polish guidelines for pharmaceutical care. The guidelines discuss the principles of: communication with patient and other health care professionals; documentation, preparation the individual pharmaceutical care plan, detection, prevention and resolving of the drug related problems; patient's education. The detailed description of the monitoring and controlling the pharmaceutical care process has also been described.
