Digital pharmaceutical documentation in practice: the role of the LekRaport application in consultations, medication reviews, and monitoring of pharmaceutical interventions

The LekRaport web application is a tool that allows pharmacists to document consultations, medication reviews, and diagnostic tests. The tool’s design allows for the collection of necessary information about the patient’s health, medications, and other health products, as well as for documenting visits and generating reports for patients and doctors. Analysis of the data collected by the pharmacist during the interview allows for the identification of medication‑related problems associated with pharmacotherapy, as well as educational needs, and for the detection of patient non‑compliance with medical recommendations or the phenomenon of non-adherence. This will provide insight into the changing needs of patients in terms of pharmacotherapy, the identification of problems, and the assessment of patients’ knowledge about the medications they are taking and their adherence to therapeutic recommendations. The tool allows for the efficient recording of patient visits and enables the collection and analysis of necessary information, which improves the provision of pharmaceutical services so that the quality of the services provided can be assessed and conclusions drawn about their further development.
